主　旨：113學年第一學期陽明校區校園餐廳滿意度調查，填問卷抽好禮！

說  明：
一、活動期間：即日起至2024年11月1日(五)止。

二、活動辦法：凡於活動期間，完成問卷並留下資料，即可獲抽獎資格乙次，由電腦進行隨機抽選。連結Google 表單方式如下：

(一)https://forms.gle/FKvPDd27FJioq49T8 (請複製後，貼於網址列上)

(二)掃描QR-Code:

三、活動贈品：

(一)「邊躺邊贏神功獎」獎：布查餐廳禮卷500元4名。

(二)「我是吃貨我驕傲」獎：BEATA 餐廳「90秒鐘，快閃夾」活動15名。

(三)「奇蹟無所不在」獎： 惠蓀咖啡禮卷200元50名。

(四)「每次都差一點」獎：7-11超商禮卷100元30名

四、注意事項：

(一)中獎者以E-MAIL通知，並於12月31日前至聯合服務中心經營管理一組領取，逾時視同放棄。

(二)逾時無人領取之獎品將併入下次評鑑活動抽獎獎品。

2. Event Details: During the event period, complete the questionnaire and provide your information to qualify for one entry into the prize draw, which will be conducted randomly by a computer. The Google Forms link is as follows:

4. Notes:

(1) Winners will be notified via E-mail and should pick up their prizes at the Joint Service Center for 1st Division of Property and Business Management before December 31. Failure to do so will be considered a forfeit.

(2) Unclaimed prizes after the deadline will be included in the next evaluation event's prize draw.
